Patient's Consent Form
    

 

Print a Patient's Consent Form.  It is available in a PDF format  which can be opened with Adobe Acrobat Reader.  For best results  use regular-size paper (8½x11") and a color printer.  In the Print dialogue box, check the option "Fit to page." 

Click the following "PDF" button to view and/or print our Patient's Consent Form.
For best results, use version 5 of the Adobe Acrobat Reader or later.
